Service Description

Complete female endocrine panel including reproductive, thyroid, adrenal, and metabolic hormones. Test between 8–10 AM. Women should test in the mid-luteal phase (approximately three-quarters of the way through your typical menstrual cycle).


Cancellation Policy

Payment and Scheduling All Longevity Metrics services must be paid in full prior to scheduling. All payments are non-refundable and final. A 5% processing and data handling fee is added to all services regardless of payment method. 10.1 Online Payment (Standard) Payment is processed through Stripe on our website, longevitymetrics.org. Scheduling should also be completed online. Credit card payment via the website is the standard and preferred method. 10.2 Mail-In Payment (Check or Cash) Patients who wish to pay by check or cash must mail payment along with a written description of the desired service to: Longevity Metrics, 3020 Carbon Place, Suite 101, Boulder, CO 80301. The 5% processing and data handling fee applies to all mail-in payments. Mail-in payments will be processed within 10 business days of receipt. Longevity Metrics will contact Patient to schedule after payment is confirmed. A $100 fee will be charged for any returned or bounced checks. 10.3 Scheduling Rules • All services must be completed withing 120 days of payment. After this time any unused funds will not be refunded. • For Your Longevity Report Card patients: The first and second, 4-hour visits must be completed within 15 days of each other. • Failure to appear or arriving more than 10 minutes late to any visit results in forfeiture of that visit's services and payment without refund. • Visits may be rescheduled without charge if done at least 10 calendar days before the scheduled date, provided scheduling deadlines are still met. • Patients with extenuating circumstances should contact info@longevitymetrics.org. Partial refunds or time extensions may be offered on a case-by-case basis at sole Longevity Metrics discretion. 10.4 Insurance and Assistance Programs Longevity Metrics does not accept medical insurance. Services cannot be provided to Medicare or Medicaid beneficiaries for services those programs would otherwise cover. Longevity Metrics does not provide ICD or CPT codes, workers compensation support, FMLA determinations, or assistance with Social Security Disability, Food Stamps, WIC, or similar programs. Medical records are available upon written request; fees may apply for records provided outside of the Consultation visit. HSA and FSA funds may be utilized for all services at Longevity Metrics.
