Three consent forms can be found at the PDF links below. Please read them. At the start of your appointment we will review and sign all of them. Signed consents are required in order to receive care at Longevity Metrics. Please complete the Pre-Visit Form prior to arrival (required for service) and bring a photo ID to your appointment. Research participation is required for all services at Longevity Metrics. Services become non-refundable when your appointment is less than 10 days away.
